At Khadeja Family Practice LLC, we want to ensure that your first visit with us is seamless and stress-free. To make the most of your appointment and facilitate a smooth healthcare experience, please take note of the following steps before coming into the clinic:
Schedule Your Appointment:
- Contact our clinic at 240-439-5619 to schedule your appointment. Our friendly staff will assist you in finding a suitable date and time that fits your schedule. Please be advised that we also offer walk in hours Tuesday to Thursday 2pm-5pm.
Complete Intake Forms:
- To expedite the registration process, you can download and print our new patient forms using this INTAKE FORM. Please fill them out accurately and bring the completed form with you to your appointment.
Insurance Information:
- We currently accept all Insurances and self-pay. if using insurance, please contact your insurance provider to confirm coverage details and any copayments or deductibles that may apply prior to scheduling your appointment. Bring your valid health insurance card to your appointment for verification.
Arrive Early:
- Plan to arrive at least 15 minutes before your scheduled appointment time. This will allow time for any necessary paperwork and ensure a timely consultation.
Insurance Co-payment or Payment Method:
- If your insurance plan requires a co-payment or if you are a self-pay patient, please have your payment ready. We accept various payment methods for your convenience.
Identification:
- Bring a valid form of identification, such as a driver’s license, passport, or state-issued ID, to your appointment for identification purposes.
Medical History and Records:
- If you have any previous medical records, lab results, or imaging reports relevant to your current health condition, please bring them with you. Your medical history will provide valuable insights into your health, and we can review them to offer the best possible care.
List of Current Medications:
- Compile a list of all the medications you are currently taking, including prescription medications, over-the-counter drugs, vitamins, and supplements. Include the dosage and frequency for each medication. This information is essential for accurate medical record-keeping.
COVID-19 Precautions:
- Please ensure that you have not experienced any COVID-19 symptoms, such as fever, cough, shortness of breath, or loss of taste or smell, in the past 14 days. Medication and Allergies List:
- Make a list of any allergies you have, including medication allergies, food allergies, and environmental allergies. This information will help us attend to your needs in an effective and safe manner.
